The short answer

The median wholesale price for Botox 100u (Allergan) across UK aesthetic clinics in early 2025 is £46 per vial, excluding VAT. However, the range is significant: 25% of clinics are paying £38 or less, while 25% are paying £58 or more. That is a 53% gap between the best and worst prices for the same product from the same manufacturer.

Why is there such a wide price range?

Several factors drive the variation in Botox wholesale pricing across UK clinics:

  • Volume: Clinics ordering 50+ vials per month typically access tier pricing unavailable to smaller buyers. Allergan's direct account tiers are not publicly advertised but can be negotiated.
  • Distributor vs direct: Some clinics buy through Allergan direct; others through distributors such as QV Medical or Aesthetics Express. Distributor pricing can be competitive but varies considerably.
  • Buying groups: Clinics in aesthetic buying groups (such as Cosmex Club or similar) may access pre-negotiated rates. However, our data suggests these are not always the most competitive.
  • Account age and relationship: Longer-standing accounts sometimes receive loyalty pricing. However, this is not guaranteed, and many long-term customers have never renegotiated.
  • Payment terms: Clinics accepting extended payment terms occasionally receive a premium pricing in return. Paying promptly or upfront sometimes unlocks better rates.

What about Dysport and Azzalure?

Botulinum toxin B products are not directly comparable to Botox by unit count, but for clinical equivalence comparisons, Azzalure (abobotulinumtoxinA, 125u vial) from Galderma has a median UK wholesale price of approximately £41 per vial. Many UK practitioners use both products and switch based on pricing — making cross-product benchmarking valuable.

Bocouture (incobotulinumtoxinA, Merz), typically available in 50u vials, has a median wholesale price of approximately £37 per 50u, though it is less widely used in UK aesthetic practice.

How to check if you are overpaying

The simplest check is to look at your most recent Allergan invoice and compare your per-vial price (excluding VAT, at standard quantities) against the benchmark above.

Your priceSignalAction
£38 or below✓ ExcellentYou are in the best quartile. Maintain the relationship.
£38 – £46~ FairAt or near the median. A polite negotiation may yield modest savings.
£46 – £58↑ Above medianWorth renegotiating. Use the P25 benchmark as a reference point.
Above £58⚠ Significantly above medianStrong case for renegotiation or exploring alternative suppliers.

Negotiation tips

If you are paying above the median, here is a practical approach to renegotiating your Botox pricing with Allergan or your distributor:

  1. 1Request a formal account review with your Allergan or distributor rep.
  2. 2State your current volume and your 12-month spend. Have this data ready.
  3. 3Reference that you are aware peer clinics are accessing better pricing for equivalent volumes. You do not need to cite Supply Index specifically — simply note that market data is available.
  4. 4Ask specifically about volume tier thresholds. Even a commitment to increase from 20 to 25 vials per month can unlock a pricing tier.
  5. 5If you are refused, ask what volume commitment would be required to access improved pricing. This opens a concrete negotiation.
  6. 6Consider whether consolidating orders (ordering less frequently in larger quantities) could help meet tier thresholds.

A note on list prices vs invoice prices

Allergan publishes list prices, but virtually no UK clinic pays list price. The prices in this article are actual invoice prices submitted by verified UK clinics — not list prices, not recommended retail prices, and not figures derived from distributor catalogues. This is what makes Supply Index benchmarks uniquely useful: they reflect commercial reality rather than published pricing.
